The CMRC license application and waiver for competition membership has been provided online. Both the application and waiver have been saved in Adobe PDF format, which will allow you to download it, print it on your printer, fill out the forms, and mail them in. Please make sure that you mail in both your application and the signed waiver. Any applications submitted without a signed waiver will be rejected.

- Print the license application and the waiver.
- Read and fill out the license application and the waiver.
- Make sure that the waiver is signed.
- Mail both the license application and the waiver to address on your license application form.
